He completed his family medicine residency with Forbes Family Medicine in Monroeville, Pennsylvania. Dr. Abroguena joined PCHC after his residency training in 2012. He is a member of the American Academy of Family Physicians and the American Medical Association. In addition to his primary care practice, Dr. Abroguena is the lead physician at PCHC Randall Square. In his free time, Dr. Abroguena loves to cook, run and travel.

Dr. Jennifer Raney earned her Bachelor of Arts in Biology from Williams College; Doctor of Medicine from Jefferson Medical College of Thomas Jefferson University; and completed her family practice residency at Brown University. Dr. Raney leads PCHC’s Nurse Practitioner Residency with 16+ years of educational and clinical experience in diverse settings including academic, community health centers, private practice, in-patient care, after-hours urgent care, addiction medicine, and student health clinics. She has mentored and instructed medical students, residents, and nurse practitioners in clinical education at Brown University and Boston University. Dr. Raney is committed to working with marginalized and underserved populations. Raised in Georgia, she is now a very happy New Englander who enjoys spending time with her husband and two sons, running, reading fiction, and being active outdoors.

Kristen Hylan received her Bachelor of Science in Nursing from Quinnipiac University and her Master of Science in Nursing - Family Nurse Practitioner, from University of Massachusetts, Boston. Prior to joining PCHC in 2020, Kristen provided holistic care in Southeastern Massachusetts. Kristen is an International Board Certified Lactation Consultant and has a special interest in obesity medicine. She is an adjunct faculty member with the Nurse Practitioner program at Simmons College in Boston, and her passion for teaching young providers led her to PCHC’s Nurse Practitioner Residency. In her free time, Kristen enjoys travel and adventure with her family.

While studying to become a Family Nurse Practitioner, Yvette completed clinical rotations in adult, pediatric, and newborn primary care with Providence Community Health Centers. She joined us from Lifespan/Coastal Medical, where she worked as a Family Nurse Practitioner at Hillside Family Medicine. Previously, Yvette worked for Bayada Home Health, The Providence Center, and Rhode Island Hospital. Yvette is a member of the American Academy of Nurse Practitioners. Yvette enjoys cooking and baking new recipes, spending time outdoors, and exploring New England with her family and rescue dog. She is fluent in Spanish.

During her academic medical career, she was recognized as Intern, Junior Resident, and Senior Resident of the Year. She was also awarded Top Trainee Teacher through Case Western School of Medicine and the Society of Teachers of Family Medicine’s 2022 Resident Teacher Award. In her free time, Dr. Fam Enjoys hiking, traveling, learning new languages, exploring a vast array of cultural foods, spending time with her two cats.

Prior to joining PCHC in 2017, Mofoluso provided care for patients with end stage renal disease in a dialysis facility and she participated in clinical rotations throughout Rhode Island. When not working, Mofoluso enjoys spending time with her family, traveling and watching documentaries on nature.

Karen Ng earned her Bachelor of Science degree in both Sociology and Biology from State University of New York at Binghamton and Medical Degree from SUNY Syracuse College of Medicine. Dr. Ng completed her family practice residency at Memorial Hospital in South Bend, Indiana. She joined PCHC in 2001. As a Clinical Assistant Professor with the Warren Alpert Medical School at Brown University, Dr. Ng enjoys teaching medical students during their Family and Community Medicine rotation. She is on the provider advisory council for Reach Out and Read RI, and involved with her church and community teaching Sunday school and volunteering with a local soup kitchen. Dr. Ng is married and has twin daughters. She wakes early each morning year-round to jog three miles, and enjoys cooking, hiking and traveling.

She practiced as a Registered Nurse and then as a Family Nurse Practitioner for several years before joining PCHC in 2018. Tenesha enjoys instructing undergraduate nurses and graduate nurse practitioner students in primary care and prior to her nursing career she taught high school Biology. Tenesha loves her work and is happy to have the opportunity to give back to the community teaching and caring for others. When she is not working, Tenesha enjoys reading, going to the gym and spending time with friends and family.

After completing her residency at the Brown University School of Medicine affiliated site at Memorial Hospital of Rhode Island, Dr. Duarte completed a Master of Science in Epidemiology and a Maternal Child Health Fellowship. Prior to joining PCHC in 2018, Dr. Duarte was the Assistant Program Director for the Brown University Family Medicine Residency and she also taught medical students. In her free time, she enjoys being outdoors and volunteering with a local soccer club.

Courtney Jusino received her Bachelor of Arts in Psychology, Bachelor of Science in Nursing, and Doctorate of Nursing Practice –Family Nurse Practitioner from University of Rhode Island. Prior to joining PCHC’s Nurse Practitioner Residency program in 2019, and deciding on a career in primary care, Courtney had an extensive nursing career. She worked at Women & Infants Hospital and Kent County Hospital in a variety of nursing roles including intensive care, neonatal intensive care, post-operative care and oncology. Courtney grew up in Rhode Island and is now raising her family across the street from her childhood home. When she’s not working or caring for her family, Courtney sits on the board for The Partnership to Reduce Cancer in Rhode Island. In her free time, she enjoys spending time with her family and shopping.

After obtaining her Bachelor of Science in Nursing from Chamberlain College of Nursing, Takeysha Duarte earned her Master of Science in Nursing with a specialty in Family Practice from Endicott College. In 2018, Takeysha joined the first class of nurse practitioners in PCHC’s Nurse Practitioner fellowship program. Previously, she spent more than a decade in nursing at Caritas St. Elizabeth Medical Center in Boston, providing care for critical care medical and surgical patients on the Intensive Care and Stepdown Units. Takeysha enjoys working with patients who bring both diverse diagnosis and background to the health center. Takeysha enjoys cooking, traveling, and exploring Rhode Island’s wonderful restaurants.

Dr. Matthew Perry received their medical degree from the Warren Alpert Medical School of Brown University, where they were a member of the Gold Humanism Honors Society. Dr. Perry completed a residency at Memorial Hospital Family Care Center and worked doing street-based medicine in partnership with House of Hope and Project Weber-Renew.
They graduated from the Premedical Postbaccalaureate program at Colombia University and received their Bachelor of Arts in Mathematics from Wesleyan University.
As an active member of the community, Dr. Perry has been involved in local activist organizations such as Showing Up for Racial Justice and the Alliance to Mobilize Our Resistance Network. In their free time, Dr. Perry likes to go to punk shows, queer events, and likes to hike, cook, and read fantasy novels.

Wesley Smyth, FNP, received his Bachelor of Science in Ecology and Evolutionary Biology from the University of Arizona before graduating from Oregon Health and Science University in Oregon with his Bachelor of Science in Nursing. He received his Master of Science in Nursing from the University of Massachusetts Boston. Prior to returning to nurse practitioner school, he was an emergency department nurse for eight years at various hospitals in Massachusetts, California, Wisconsin, and North Carolina.
Wesley joined our Nurse Practitioner Residency Program in 2023. In his free time, Wesley enjoys exploring Rhode Island with his family and his dog Lucy.

Long Yang, FNP, received his Bachelor of Science in Nursing (graduating Magna Cum Laude) and his Master of Science in Nursing from the University of Rhode Island. Prior to joining PCHC’s Nurse Practitioner Residency Program in 2023, he had 10 years of experience as an RN at The Miriam Hospital. He is a member of the Hmong Nurses Association and volunteers as a member of the Hmong Parish of St. Michael’s Church in Providence. Long is fluent in Hmong and English. In his free time, Long enjoys cooking, fishing, sports, singing, music, and traveling.
